Life Navigators offers in-home and community-based services to children, youth, and families found eligible for CLTS Services. The program coordinates and facilitates access to formal and informal services and supports that assist children and youth with disabilities to live a safe, happy, and healthy life in our community.

Creating The Best Support For Kids With Disabilities And Their Families

Started in September 2023, in partnership with Milwaukee County Department of Health and Human Services – Children, Youth and Family Services, Life Navigators began delivering the Children’s Long-Term Support Waiver Program (CLTS). Our experienced Program Manager and Support and Service Coordinators will help connect kids with disabilities and their families with the services they need to live a happy and healthy life.

Support and Service Coordination

Our Support and Service Coordinators team will assist children with disabilities and their families to achieve an inclusive, interdependent, and self-empowered life. Service includes connecting families to support throughout the community, finding ways to reach each child’s individualized goals, and promoting self-determination and involvement in all facets of life.

Services funded through CLTS are authorized based on the assessed needs and outcomes. Examples of services that will support children and their families include:

  • Home modifications
  • Respite care
  • Therapeutic services
  • Transition services
  • Adaptive aids
