Programming Changes Bring New Opportunities

In December 2009, Life Navigators, in partnership with My Choice Wisconsin (MCW), began delivering the Family Care benefit to individuals with disabilities throughout Milwaukee County. Our Care Management Unit (CMU) team of Care Managers, Registered Nurses, and Lead Supervisors, over the past fifteen years, proudly served hundreds of CMU members and their families.

Care Management Unit: Contract Change
In January 2023, MCW informed Life Navigators that our CMU contract would end. MCW is in the process of being acquired by Molina Health Care and will be bringing all CMU services internally. Life Navigators’ CMU contract officially ended on May 31, 2023. We are thankful to our dedicated team that impacted so many and for our partnership with MCW. We are now working to ensure a smooth transition for all CMU members and their families.

Core Services: Program Expansion
While our CMU services are wrapping up, Life Navigators’ other core services continue to expand. We recently added a new program coordinator to our Guardianship team and will be expanding our Trust & Futures Planning services in the coming months. Supporting individuals with disabilities to be out and active, we are seeing a record number of consumers participate in Self Advocacy & Independence programs and look for us out in the community as Information, Education & Outreach services are growing. Our Family Support program is working with families both in-home and out in the community and our caregiver groups continue to grow.
